Applicants must be registered to take this test, either at the time of successful completion of both the APCAT and ACT exams or upon being instructed by a member of the Recruit Selection Unit.

This is the medical screening questionnaire that candidates are required to complete prior to running the A-PREP - PAR-Q+.  A completed PAR-Q+ document must be provided to the test facilitators at the start of the A-PREP session - it should not be submitted with the Application.

Also, a completed Applicant Consent for A-PREP Testing and Release of Information document must be handed at the start of the A-PREP session.  Again, it should not be submitted with the Application.

If you have any medical concerns, for instance high blood pressure or shortness of breath, or you have checked the 'Yes' box to any of the PAR-Q+ questions, you should have both the PARMedex and APREP Medical Clearance Form completed and signed by a Physician prior to your testing date; you must then bring them along to the session - do not submit them with the Application.

Failure to bring the appropriate completed documents may result in candidates not being permitted to participate in an APREP testing session.

Do not submit any of these medical forms with your Application - they are to be brought with you to the APREP testing session.
