Application Checklist

Originals are required of the following reports:

• Handwritten Personal Disclosure Form
• Completed Security Clearance Declaration
• Completed Vision Report
• Completed Supplemental Vision Report (if required)
• Completed Hearing Report
• Completed Authorization for Release of Information

Copies are required of the following:

• High School Diploma or GED and official transcripts or for international applicants, IQAS certificate
• Post Secondary Diplomas/Degrees and official transcripts, if applicable
• Current First Aid and CPR Certificates
• Driver's Licence
• Driver's Abstract (dated within one month)
• Pardons, if applicable
• Proof of Citizenship / Residency (Birth Certificate, Citizenship or Permanent Resident  documentation, etc.)

Please note that at a point in the selection process (usually prior to interview), applicants will be asked to provide five written letters of reference (in addition to the references listed in the application form) and a typewritten resume.
